Do you know of an individual that demonstrates outstanding service to the Village of South Holland? Now is the perfect opportunity to nominate residents who believe in, and show a commitment to South Holland's values of Faith, Family and Future.
The Faith Leadership Award is presented to a resident who has demonstrated outstanding service to the faith community. In addition to the nominator, a congregational leader must also sign the nomination. Faith Leadership Nomination Form
The Family Leadership Award honors a resident who has demonstrated outstanding service as a role model to married couples, families or children. Family Leadership Nomination Form
The Future Leadership Award is given to an elected official, employee or volunteer who has demonstrated a commitment to improving the future vitality of the Village. Future Leadership Nomination Form
The deadline to submit nomination forms is December 31, 2006.
Awards will be presented at the Volunteer Appreciation Dinner in
February 2007.
These nomination forms should be printed first, then filled out by hand or typewriter.
